Practical steps to reduce med errors

 By focusing on five key rules of medication therapy, clinicians can reduce the occurrence of medication errors and improve patient safety. These key rules include the right drug, right dose, right route, right time, and right patient. Once we are satisfied that we have met what most would consider to be the most basic requirements of medication therapy, we can begin to focus on more complex matters such as allergic reactions, hypersentive reactions, etc. Other areas of concern can occur where one can make an error of omission (failure to act correctly) or an error of commission (acted incorrectly).
